Operational principle
A rapidly-moving projectile transfers its energy to the treatment head. The generated radial shockwaves are spread out and absorbed by the area of the body that is being treated.

Shockwave therapy associated with cryotherapy (thermal shock) improves comfort of the patient, makes the therapy softer and increases the effectiveness.

Shockwave therapy can be uncomfortable. There is also the risk of some side effects. The use of the cryotherapy BEFORE the shockwave treatment is beneficial due to:
oIts analgesic effect which makes the treatment completely painless and offers more comfort for the patient. Thus it is possible to use suitable parameters which improve the effectiveness of the therapy.
oIts vasomotoric effects decrease the risk on bruises caused by shockwave therapy.
oIts Hunting reaction effect accelerates the healing process of shockwave therapy.
First apply cryotherapy with the ShockMaster ICE CT:
The ShockMaster ICE-CT cools down surrounding air but is on top of that the first cryotherapy device to do this with an infra red system, thereby ensuring a constant temperature on the treated area.
An intelligent hand piece not only measures the temperature on the treated area, but adjusts the airflow automatically in accordance with the temperature reached on the Skin (biofeedback system). This enables the therapist to setup his t the therapist situates the patient in a comfortable position, positions the treatment head on the required area, fixes the temperature and from then on the ShockMaster ICE CT will automatically adept its airflow in order to reach it continuously.
Immediately afterwards the therapist start to apply shockwave therapy with the ShockMaster 500.
Application of the therapy
The therapy can be split up in 2 phases
Depending of the type of indication, the therapist will choose which frequency, bar and amount of pulses he will use. The therapist localizes the pain region and applies gel. Then he releases shockwaves as the hand piece is moved over the entire pain region in circular motions.
In the next stage, the therapist will treat the muscle region related to the indication, also called as D-ACTOR(R) application. This application represents a new therapy approach. The combined use of radial shockwaves and vibrations allows effective local trigger point treatment as well as activation of muscle and surrounding connective tissue.

体外冲击波治疗多种疼痛
体外冲击波疗法在骨科康复中的临床应用

体外冲击波疗法(Extracorporeal shock wave therapy, ESWT)是一种应用体外冲击波治疗仪产生冲击波能量，通过探头将能量传递至体表，再透过软组织聚焦于治疗部位的方法，该法具有安全有效、非侵袭性、并发症少的等优点，它作为一种绿色治疗方法，已经被广泛地应用于骨科康复领域的多种疾病，并展示了广阔的应用前景。
体外冲击波最早应用于临床是用来治疗泌尿系统的碎石，对进行多次体外冲击波碎石的患者进行泌尿系彩超检查时无意中发现部分患者经过冲击波治疗的肾脏体积比对侧大一些，使人们想到冲击波在一定能量范围内可能促进组织生长。20?世纪80年代，临床研究人员开始应用冲击波作用于患者的骨折不愈合部位，发现体外冲击波有促进骨折愈合的作用。此后，人们加快了对体外冲击波在骨科康复中临床应用的研究，现介绍如下。
一、体外冲击波的工作原理及其作用机制
1．体外冲击波的分类及工作原理
根据冲击波波源产生的形式不同，体外冲击波分为以下4种：气压弹道式冲击波（属于放射式冲击波）和液电式、电磁波式及压电式冲击波（均属于聚焦式冲击波）。
其中，气压弹道式冲击波治疗机是通过自由移动的探头，将由气压弹道产生的冲击波以放射状扩散的方式传递至治疗部位，达到治疗的目的，而后三种均属于传统的体外冲击波，是通过发射体将能量聚焦于治疗部位而起到治疗作用。根据骨肌疾病的特点，气压弹道式冲击波即放射式冲击波更适宜。
根据冲击波能量的大小不同，体外冲击波通常分为高、中、低3个能级：低能量冲击波：0.06-0.11mJ/mm2?；中能量范围：0.12-0.24mJ/mm2?；?高能量范围：025-0.39mJ/mm2?。
2．体外冲击波的作用机制：
（1）理化效应：①机械效应；②镇痛效应；③成骨效应。
（2）生物效应：①空化效应；②应力效应；③压电效应。
二、体外冲击波疗法适应证及禁忌证
1．体外冲击波的适应证：
（1）软组织慢性损伤性疾病：急、慢性损伤性疾病导致的疼痛、肩周炎、肩峰下滑囊炎、肱二头肌长头腱炎、钙化性岗上肌腱炎；网球肘、肱骨内上髁炎；髋部及膝部慢性损伤性疾病，如弹响髋、跳跃膝(胫骨结节骨骺骨软骨炎)、膝关节炎；足跟痛及足底筋膜炎等；腰肌劳损、急慢性腰软组织损伤。
（2）骨组织疾病：股骨头坏死、骨折不愈合或延迟愈合、小于0.5cm的骨缺损。
（3）其他：外科术后或软组织粘连挛缩导致的关节僵硬。
2．体外冲击波的禁忌证：
(1)?全身性因素：存在严重心脏病、高血压患者；安装心脏起博器患者；血栓形成、骨骺未闭合的儿童、使用抗免疫药剂的患者；患有出血性疾病、凝血功能障碍、各类肿瘤患者；孕妇。
（2）局部因素：关节液渗漏的患者；冲击波焦点位于脑及脊髓、大血管、重要神经干走行处及肺组织者；肌腱及筋膜急性损伤；骨组织萎缩及感染性骨不连、局部感染及皮肤破溃患者；大段骨缺损者。
三、体外冲击波疗法在骨科康复领域的应用举例及优点
1．体外冲击波疗法在骨科康复领域的应用举例：
（1）冲击波治疗骨折不愈合或延迟愈合：治疗时应从低能量起始，逐渐增至所需的治疗能级，具体治疗方案：每1cm?的裂隙长度需要?500-800?次的高能体外冲击波治疗，一般每次治疗需要冲击??次，能流密度为?0.04-0.28mJ/mm2，在治疗过程中，应定时使用X线进行影象跟踪保证聚焦准确。
（2）冲击波治疗跟痛症：跟痛症是跟骨结节的反应性骨刺样成骨现象，体外冲击波治疗跟痛症的目的是治疗肌腱和筋膜的慢性炎症，而并非使外生性骨疣解体。宜用底到中等能量治疗跟痛症，应逐渐提高能量到所需水平，能量密度为?0.08-0.14mJ/mm2?。
（3）冲击波具有促进组织代谢?.?止痛和组织修复的作用：在治疗一些慢性软组织疾病方面也表现出良好的疗效，原理是冲击波可加速骨与肌腱连接部位的血管新生化，从而达到治疗效果，多用于治疗肱骨外上髁炎、网球肘、肩周炎等疾病。
2．体外冲击波的优点在于：
（1）损伤轻微,可替代某些外科手术疗法；
（2）一般采用简单麻醉或不必麻醉；
（3）治疗时间短，风险小，可在门诊进行治疗；
（4）无需特殊术后处理，且术后恢复较快；
（5）治疗费用远远低于开放式手术。
