支气管哮喘合并侵袭性肺曲霉菌病1例报告
【摘要】:正支气管哮喘是一种异质性疾病,通常以慢性气道炎症为特征,肺部感染为其急性发作的主要诱因。肺曲霉菌病的发病率近年呈增高趋势[1],主要原因为广谱抗生素、糖皮质激素等免疫抑制剂的广泛应用和诊断技术的不断提高。其中侵袭性肺曲霉菌病(invasive pulmonary aspergillosis,IPA)危害最大、病死率最高。现回顾分析复旦大学附属中山医院呼吸内科近年收治的1例支气管哮喘合并IPA患者的诊治过程及临床特点,旨在提高对该病的认识。

Objective To investigate the clinical effect of atomizing inhalation of ambroxol hydrochloride in adjuvant treatment of infantile pneumonia.Methods 258 cases children with pneumonia were selected as research object,who were divided into 2 groups randomly.Control group(129 cases) was treated by conventional measures.Observation group(129 cases) was treated by inhalation of ambroxol hydrochloride on the basis of routine treatment.The clinical effect was then compared.Results The total efficiency of observation was 89.92%,and the group B was 69.77%.The difference had statistical significance between by comparisonn(P
0.05).Cough disappeared time,lung wet rales disappearance time and body temperature recovery time of the observation group were shorter than the control group(P
0.05).Adverse reaction incidence of the observation group was 6.2%,and the group B was 5.43%.There was no significant difference by comparisonn(P
0.05).Conclusion Atomizing inhalation of ambroxol hydrochloride improve the effect of pediatric pneumonia,which is worthy of application.

Ionic mechanisms of cell volume regulation.1, , .1Department of Medicine and Liver Center, Yale University School of Medicine, New Haven, Connecticut 06510.AbstractIsolated hepatocyte suspensions were exposed to hypotonic and hypertonic stresses and serial cell volume measurements were made with an electronic particle size analyzer. With the exposure to hypotonic (160 mosM) buffer, hepatocytes swelled within 30-60 s as osomometers [relative volume (RV) = 1.44 +/- 0.08] and subsequently underwent regulatory volume decrease (RVD) back toward the resting (isotonic) level (1.16 +/- 0.05). This volume recovery was blocked by 65 mM extracellular K+ concentration and inhibited by barium (1 mM) and quinine (0.5 mM) but not by bumetanide (0.1 mM). Chloride depletion inhibited RVD by approximately 40% while 0.5 mM 4,4'-diisothiocyanostilbene-2,2'-disulfonic acid (DIDS) blocked the recovery by almost 90%. Calcium deprivation had no effect on RVD, nor did ouabain, amiloride, or sodium replacement. When exposed to buffer made hypertonic by addition of 200 mM sucrose, cells shrunk as osmometers (RV = 0.74 +/- 0.02) but did not exhibit regulatory volume increase (RVI). However, when cells that had first undergone RVD were reexposed to isotonic medium (relative hypertonic stress) RVI could be demonstrated from RV 0.77 +/- 0.17 to 0.91 +/- 0.20. This response was dependent on sodium, partially dependent on bicarbonate and chloride, and inhibited by the Na(+)-H+ exchange inhibitor amiloride (1 mM) but not by DIDS. Our findings suggest that RVD in rat hepatocytes is mediated by quinine- and barium-sensitive K+ conductance and DIDS-sensitive anion conductance, which is partly accounted for by Cl-; RVI is mediated by activation of Na(+)-H+ exchange coupled with a bicarbonate- and chloride-dependent but DIDS-insensitive process.PMID: 2305895
