腰麻药中加入微量吗啡用于剖腹产术后止痛
硬膜外腔注入阿片类药物已广泛用于产科术后镇痛治疗,而蛛网膜下腔注药临床却很少有报道。本文对40例剖腹产病人蛛网膜下腔注入微量吗啡,对其术后镇痛效果、安全性及其并发症的防治进行了研究。 1资料与方法 1.1一般情况:40例足月妊娠产妇腰麻下行剖腹产术决SAI级,年龄22一25岁,体重55ee守3电,身高15仆,17压.。病种包括:胎儿宫内窘迫16例,头盆不称14例、疤痕子宫5例,羊水过少4例,巨大儿1例,急诊手术28例,择期手术12例。 1 .2药液配制:吗啡用生理盐水稀释至0.1啤/回,腰麻药液为:0.5%布比卡因2耐十吗啡液2创(0.2瑰)+生理盐水4成,总量为腼.。 1.3方法:术前半小时肌注苯巴比妥钠100吨、阿托品0,5毗。入室后,立即开放静脉通路,快速输液300一50压川,取右侧卧位,环一3间隙穿刺,有脑脊液回流后,注入配好的布比卡因吗啡液8而,60秒内注完,立即平卧,手术台左倾15度。术中监测血压(Bp)、脉搏(p)、呼吸(R)、脉搏氧饱和度(s川〕2)、胎儿娩出后记录1而n及5而nA此ar评分。10例病人术毕吸室内空气条件下,作动脉血气分析。......(本文共计2页)
