常见妇科病“白带”辩析

女性从青春期开始便有少量清亮乳白色分泌物从阴道流出，随年龄的增长，阴道流出的液体逐渐增多，这种流体称为白带。正常白带是从输卵管、子宫、子宫颈及阴道等处所排出及渗出液体的总称，主要成分是阴道上皮的渗出液，子宫内膜及子宫颈腺体，输卵管内膜上皮等处的分泌物，以及该处剥脱的上皮细胞和少量白细胞，微生物等，正常白带呈液状，透明白色或乳魄，可以滑润阴道，保持阴道前庭区的湿润并产生舒适感，对粘膜及皮肤无刺激性。白带具有轻微的腥味但无臭味，其质和量随月经周期的变化略有不同。
　　当患有某种妇科病时，白带的量和性状就会发生变化，从白带的不同变化可初步判断患有哪种疾病以及病变的性质属良性还是恶性。下面列举常见的几种妇科病的白带变化。
　　婴幼儿阴道炎
　　婴幼儿时期内外生殖器尚未发育成熟，体内缺少雌激素，阴道上皮薄，细胞内缺乏糖原，酸度低，抗感染力弱，易受细菌感染发生阴道炎。1岁-5岁的幼女尤为多发，主要表现为阴道流液呈脓性并有臭味，阴道前庭区及外阴部潮红。由于外阴痒痛，患儿经常用手抚摸，搔抓，有时粘膜破溃并出现细小裂伤，这时母亲应带孩子到医院治疗，如果婴幼儿的阴道流液为淡粉色或血水样，甚至流血，这时需经妇科医生进一步检查以排队子宫颈或阴道肿瘤。
　　细菌性阴道病
　　细菌性阴道病是生育年龄妇女最常见的阴道感染之一，是由以厌氧菌为主的多种病原体引起的一种无阴道粘膜炎症表现的综合征，主要症状为白带增多，魄，较衡薄并伴有鱼腥臭味，过去认为本病属于一种非特异性阴道炎或加特纳氏阴道炎，但由于这种病并无阴道炎症表现而白带的变化却非常明显，因此称为细菌性阴道病。在白带涂片中可以查到线索细胞，阴道液GP值增高到4.5以上，胺试验阳性为其特征性改变。
　　老年性阴道炎
　　多见于绝经后的妇女或手术切除双侧卵巢或曾行盆腔放射治疗后致卵巢功能衰退者，因体内雌激素缺乏，阴道粘膜易受细菌感染而发生炎症。主要表现为白带增多，呈黄水状，感染严重时白带为腔性有臭味，阴道粘膜发生表浅溃疡时白带可呈血性或出现点滴出血。由于白带增多，可刺激外阴部产生外阴炎及外阴瘙痒，对于绝经后的老年妇女，有血性白带或点滴出血时，首先应做工阴道细胞学检查以除外子宫颈的恶性病变。
　　输卵管病变
　　患有输卵管病变时白带呈稀水样一阵一阵从阴道口向外流出，不伴有腹痛或其它不适，如果流出的是魄清亮的水样液，可能为输卵管积水；如果流出的液体混有血液又不那么清亮，则有可能为输卵管的恶性病变。
　　滴虫性阴道炎
　　滴虫性阴道炎是由毛滴虫感染引起的。滴虫属厌氧的寄生原虫，适于在阴道内生长繁殖而发生炎症。典型的白带呈淡黄，灰贡或乳白色，较稀薄，量多，并有臭味或鼠臭味，含有细小的泡沫，有时混有血液呈粉色脓性。分泌物多时常溢出阴道外口，刺激外阴部皮肤，患者感到外阴瘙痒、灼热。合并尿路感染时可有尿频，尿痛甚至尿血，分泌物中有泡沫是因为碳水化合物在阴道内被分解后产生所体所致。
　　霉菌性阴道炎
　　霉菌性阴道炎约有80%-90%是由白色念珠菌感染所致，其它10%-20%是由其它念珠菌或球拟酵母菌属感染而发病。主要表现为白带量明显增多，呈乳白色，较粘稠，常为豆渣样或凝乳状；有时较稀薄，内含有白色小碎块，病情较重者小白块大量附着在阴道壁上不易擦下，强行擦除后基底部出现受损的糜烂面或表浅溃疡。大量白带刺激下可产生外阴及阴道部瘙痒及灼热感，症状严重时坐立不安。
　　淋病是性病中最常见的一种，是由淋病双球菌引起的一种传染病，几科全部都是由性交传播感染。与男性淋病患者性交后感染高可达60%-90%；少数是通过受淋球菌污染的衣物，浴巾，便器，手指，医疗器械等间接传染，从接触病原菌到发病，潜伏期为2天-10天，平均只有3天-5天，首先受感染的部位是生殖泌尿道粘膜表面，因此主要表现为白带增多，有多量腔性或液脓性分泌物由阴道口流出，并有淋菌性尿疲乏炎表现，尿道口红肿，压痛，尿道口有脓性分泌物，尿频，尿急，排尿时有烧灼样痛，手指伸入阴道下段由内向外按压阴道前壁时由尿道口可挤出脓性分泌物，外阴，阴道均有不同程度的红肿及压痛。
　　阿米巴性阴道炎
　　本病常发生于患有阿米巴痢疾的患者，由于其大便中的阿米巴滋养体随粪便排出时粘染外阴部，若侵入阴道内则引起阴道炎，阿米巴进入阴道后侵入阴道粘膜，并分泌虫溶组织酶使阴道上皮细胞坏死形成溃疡，因此白带量增多呈浆液性，混血或黄色粘液脓性，有时白带中含有黄棕色坏死组织的细小碎末呈酱汤状。
　　妇科肿瘤
　　绝经前后的妇女，白带量较多，呈海米水样，混浊并有臭味，或白带混血呈淡粉色或有点滴出血，应考虑为子宫颈，阴道或子宫体的恶性病变，应到医院做进一步检查。某些良性病变有时亦可有白带增多混有少量血液如子宫颈息肉，粘膜下子宫肌瘤，子宫颈糜烂，子宫内膜炎，宫内节育器引起的副反应等，均应经医生检查后给予诊断和处置。
