AbstractPurpose &to evaluate the effectiveness of distinguishing diagnosis of adrenal adenoma from malignant tumor using CT attenuation values in different scan states.Materials and methods &31 adrenal adenomas in 28 patients examined with non-enhenced scan, enhenced scan and 1-hour-delayed enhanced scan.; The CT values of& tumors& in& different& scan& states& were& calculated and statistical analisis were carried. Results &&Adenoma 19, metastatic& tumor 5, adenocarcinoma 4.On non-enhanced& scan, the mean CT value& of adenoma was 5.510Hu versus 3313.8 for malignant tumor (p&.001); At a threshold value of 19 Hu, specificity and positive prediction value for the diagnosis of adenoma were 100% with a sensitiviy of 85%; During 1-hour-enhenced scan , the& mean CT value was 1211.5 Hu for the adenomas versus 479.8Hu for the malignant (p&.001); At a& threshold value of 30 Hu, specificity and positive prediction value for the diagnosis of adenoma were 100% with a sensitiviy of 90%; A much greater overlap of& CT values occurred on enhenced CT scan with a low sensitivity between& adenoma& and& malignant tumor.conclusion& Unenhenced or 1-hour-delayed CT attenuation values can distinguish between adenoma and& malignant tumor.but the& former method was& much& simpler and cheaper.&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&& &&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&Key wordsA Dist CT attenuation value&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&&
